Personalized nutrition and diet support for clients receiving home care packages from a dietitian

Case study: Personalised Nutrition Support from a Home Care Package Dietitian

When seeking a home care dietitian on the Gold Coast, this 80-year-old woman and her nursewanted dietitian support. Despite having regular carers and domestic health, she struggled to maintain a healthy weight and lacked the energy to prepare balanced meals. Concerned about her nutritional status, her nurse sought the expertise of a home care dietitian to address her weight loss and improve her overall health.

The Challenge: Managing Weight Loss at Home

During the initial home visit, our Gold Coast dietitian assessed her nutritional status and identified several challenges. Her recent weight loss was impacting her energy, immune system, and overall well-being. She needed a tailored nutrition plan to regain weight, prevent malnutrition, and enhance her quality of life. Given her limited mobility, she also required meal solutions that were easy to prepare and fit her dietary needs.

Our Solution: Personalised Nutrition Support

Our home care dietitian worked with this particular client to create a personalised nutrition plan focused on addressing her weight loss and nutritional deficiencies. Key interventions included:

  • High-calorie, nutrient-dense meals (provided in her normal small portions) that contributed to healthy weight gain and prevented further muscle loss.
  • Strategies to increase protein intake to support muscle maintenance and immune function.
  • Simple, easy-to-prepare recipes that she could manage with her limited mobility.
  • Education on meal timing and portion sizes to ensure she was consuming enough calories throughout the day.

Ongoing support:

The dietitian suggested simple meals she could prepare herself.  Regular follow-ups with the dietitian ensured continued progress and adjustments to her meal plan.

Conclusion:
With the support of a home care dietitian, this client was able to address her malnutrition risk. The dietitian continued to review her intake and weight with her nurse.
